Turin Forks Landscape Contracting, Inc. on behalf of MICHAEL GIACONE requests an Amendment
to Wetland Permit#7188 to extend the existing groin 20' landward to prevent further erosion; install
a k20 linear foot boulder revetment extending from the south east corner of dwelling to west side of
groin consisting of 1-2 ton stone armoring and rip rap base; and a total of 50 cubic yards of clean
sand fill to be added to replenish dune over septic system; and entire dune will be re-vegetated with
native species (i.e. beach grass, bayberry) plantings. Located: 360 Park Avenue Extension,
Mattituck. SCTM# 1000-123-8-28.1
The proposed action has been reviewed to Chapter 268,Waterfront Consistency Review of the
Town of Southold Town Code and the Local Waterfront Revitalization Program (LWRP) Policy
Standards. Based upon the information provided on the LWRP Consistency Assessment Form
submitted to this department, as well as the records available to me, it is my recommendation that
the action is CONSISTENT with the Policy Standards and therefore is CONSISTENT with the LWRP
provided that the Board considers the following:
1. Clarify the top elevation of the revetment.
2. The potential deflection of wave energy to_the property-to the east of the parcel should
be assessed.
3. The replacement of the existing septic system shown on the plan with an All OWTS is
strongly recommended to further Polices 5 and 6. Grant funding is available to cover
the installation costs of the system through the http://www.reclaimourwater.info/
website.
pursuant to Chapter 268, the Board of Trustees shall consider this recommendation in preparing its
written determination regarding the consistency of the proposed action.

A. INSTRUCTIONS
1. All applicants for permits* including Town of Southold agencies, shall complete this CCAF for
proposed actions that are subject to the Town of Southold Waterfront Consistency Review Law. This
assessment is intended to supplement other information used by a Town of Southold agency in
making a determination of consistency. *Except minor exempt actions including Building Permits
and other ministerial permits not located within the Coastal Erosion Hazard Area.
2. Before answering the questions in Section C, the preparer of this form should review the exempt
minor action list, policies and explanations of each policy contained in the Town of Southold Local
Waterfront Revitalization Program. A proposed -action will be evaluated as to its .siUijficant
beneficial and adverse effects mon the coastal area{which includes all of Southold Town.
3. If any question in Section C on this form is answered "yes" or "no", then the proposed action will
affect the achievement of the LWRP policy standards and conditions contained in the consistency
review law. Thus, each answer must be extnlained in detail, listing both srnnportiina and non-
Moorti ng facts. If an action cannot be certified as consistent with the LWRP policy standards and
conditions,it shall not be undertaken.
