VINCENT BIALESKI, residing at (No #) Cardinal Drive,
Mattituck, New York, being duly sworn, deposes and says:
1. I was the owner of premises located at (No #)
Oregon Road, Cutchogue, New York shown on the attached
survey, for many years. My father owned it for many years
before me. The premises is currently owned by CHARLES
TYLER & MARGarET TYLER~ who Durchased said premises on June
14, 1982 from JOSEPH YABONI & MYRON HAUPTMAN.
2. I lived there for many years and the buildings, as
shown on the attached survey, were constructed many years
prior to 1957. The premises contains of a two-story frame
dwelling, frame gargage, two frame buildings and a shed.
3. To my knowledge, there have been no changes or
alterations made to the buildings that would require a
Building Per~it or Certificate of Occupancy, since 1957.
I make this application in support of the Tyler's
application to the Town of Southold for a Certificate of
Pre-Existing Use for the dwelling, garage, two frame
buildings and shed.

Instructions
A. This application must be filled in Wpewriter OR ink, and submitted m ~ to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal-{S-9 form or equal),
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non-conformin~ uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of p~operty showing all property lines, streets, buildings and unusual natural or
topographic features.
2.Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.
