HOUSING CODE INSPECTION

Upon receipt of an application for a Pre-existing Certificate
of Occupancy, I made an inspection of this one and one-half story,
wood framed dwelling and the accessory structures on the premises.
I picked up key for the dwelling from the Matt Agency, Main Road,
Mattituck, and began this inspection at approximately 2:15 P.M.
The first floor of dwelling consists of a kitchen, dining
room, living room, full bathroom, and three bedrooms. The front
entry to building is protected by an open porch. The entry is to
a vestibule which is off the kitchen. There is an enclosed porch
on the west side of building which is accessible from the dining
room. The rear of the building has an entry to the living room.
This is protected by an open porch. The second floor, which is
an unfinished attic, contains a finished bedroom which is heated.
Heat is supplied to all rooms on the first floor from an oil -fired
steam heat furnace which is located in the full cellar of building's
cement block foundation. Accessory structures consist of a one -
car garage which is in the front yard area and a boat house in the
rear yard area.
The following violations of the Housing Code, Chapter 52,.
Town of Southold, New York, were found:
Front entry - porch steps are loose and deteriorating,
Article III, Section 52-30 A.
Bedroom - on the northeast corner of building, wall light
fixture is broken and the socket is loose, Article V,
Section 52-56 A (1).
Finished bedroom - in attic, sheetrock ceiling is loose and
sections are missing, Article III, Section 52-32 C.
Dormer roof - over finished bedroom, exposed section of roof
where the sheetrock is missing, bottom of the valley at dormer
roofing is rotted away. Excessive daylight is noticable.
Also, there are indications that water has entered building
at this location, Article III, Section 52-31 C.
Cellar - interior stairway, open sides, no railings, Article
II, Section 52-27 B.
Furnace - no cover on stack switch, Article V, Section 52-56 A (1).

Open porch - on south of building, end posts on east and
west corner which support the roof, bearing joists on
cement block pier foundation are rotting away and causing
deformation in the deck at the point where the posts
supporting the roof are located, Article V, Section 52-30 A.
Accessory structure - one -car garage in front yard area,
rear wall of the garage and the sill have been pushed away
from the foundation of building on the northwest corner,
Article III, Section 52-31 A & B.
Boat house - in rear yard area, approximately 50% of the
building has collapsed and the remaining section shows
evidence of deformation. Siding and roofing is missing,
Article V, Section 52-30 A & B; Article VI, Section 52-62 B.
Overhead electric power lines - from the east side of
dwelling run to the boat house in rear yard, Article V.
Section 52-56 A (1).
Inspection completed at approximately 3:10 P.M. Key was
returned to the Matt Agency.

APPLICATION FOR CERTIFICATE OF OCCUPANCY
Instructions
A. This application must be filled in typewriter OR ink, and submitted in DUPLICATE to the Building
Inspector with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and
unusual natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal—(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and
installations, a certificate of Code compliance from the Architect or Engineer responsible for
the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non -conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of property showing all property lines, streets, buildings and unusual natural
or topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent in-
formation required to prepare a certificate.
C. Fees:
1. Certificate of occupancy $5.00
2. Certificate of occupancy on pre-existing dwelling or land use $5.00
3. Copy of certificate of occupancy $1.00
