wood framed dwelling with porch; 3 Old sheds; all situated in a 'A' Residential
Agricultural zone, with access to Skunk Lane; a Town maintained highway

APPLICATION FOR CERTIFICATE OF OCCUPANCY
Instructions
This application must be filled in typewriter OR ink, and submitted I ~ii~ to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic featu res.
2. Final approval of Health Dept. of water supply and sewerage disposal--(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.

Veronica Tramaloni, being duly sworn, deposes and states as
follows:
That ! am the owner of certain premises known as 3275 Bay Avenue, Cutchogue,
New York, designated on the Suffolk County Tax Map as District 1000,
Section 97, Block 9, Lot 10.3. Located on said property is a house, farm
stand, garage, barn, chicken coop and smal! green house. All of which
were constructed prior to Apri! of 1957. I submit this affidavit in support
of my application for a pre-existing Certificate of Occupancy for said
structures.
