tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, end unusual
natural or topographic features. /
2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.

I, IRVING L. PRICE, JR., residing at 230 Fourth Street, Greenport,
New York, being duly sworn, deposes and says:
That I am an attorney at law with offices at 828 Front Street, Greenport,
New York;
That I represent the present owner of the premises located at 522 Bailey
Avenue, Greenport, New York and designated on the Suffolk County Tax Map as
District 1000, Section 34, Block 4, Lot 9 and am familiar with the condition of same;
That the structures thereon have been designated, arranged and physically
occupied as a one-family dwelling;
That the condition of the existing structures on the premises are good;
That the structures are pre-existing prior to April 23, 1957, and they
are in habitable condition, that I know this of my own knowledge having visited and
viewed the premises many times;
That I make this affidavit in the knowledge that the Building Department
of the Town of Southold will rely on the accuracy thereon in issuing a certificate
of occupancy.
