Upon receipt of an application for a Pre-existing Certificate
of Occupancy, I made an inspection of this one and one-half story,
wood-framed dwelling. I was admitted to building by Ronald Lajda
and began this inspection at approximately 10:30 A.M.
The first floor of dwelling consists of a kitchen with an
eating area; at the end of this room there is a sliding glass door
unit that opens onto a raised deck approximately 16' by 16' in
size. Also, there is a full bathroom and a living room which opens
onto an enclosed porch on the west side of building. ~

porch has an entry on the south end from a roofed, open porch. The
second floor has four bedrooms and a full bathroom. All rooms are
heated with electric heating elements. Building is constructed on
a cement block foundation with a full cellar. Accessory structures
on premises consist of a one-car garage, which is unattached and is
located in the front yard area.
The following violation of the Housing Code, Chapter 52,
Town of Scuthold, New York, was found:
~ntr~ - to enclosed porch, no means of controlling light
room upon entering, Article V, Section 52-56 B (2).
The one-car garage in the front yard area violates Chapter 100
of the Zoning Ordinance, Article III, Section 100-32, covering
accessory structures which are required to be located in the rear
yard area.

APPLICATION FOR CERTIFICATE OF OCCUPANCY
Instructions
A. This application must be filled in typewriter OR ink, and submitted in DUPLICATE to the Building
Inspector with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and
unusual natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal--(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and
installations, a certificate of Code compliance from the Architect or Engineer responsible for
the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey.of property showing all property lines, streets, buildings and unusual natural
or topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent in-
formation required to prepare a certificate.
