Instructions
A. This application must be filled in typewriter OR ink, and submitted in duplicate to the Building Inspec-
tor with the following; for new buildings or new use:
1, Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2, Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal).
3, Approval of electrical installation from Board of Fire Underwriters.
4, Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of peoperty showing all property lines, streets, buildings and unusua~ natural or
topographic featu res.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.

says:
HERMAN STRICKLAND, being duly sworn, deposes and
I reside at Middle Road, Mattituck, New York.
That the premises shown on the annexed survey made
by Otto W. Van Tuyl & Son and dated May 25, 1961, and made a
part of this affidavit by reference, accurately represent the
premises as they existed prior to 1957.
The various structures shown on the said survey
were in existence prior to the year 1957.
No other structures, as defined by the Zoning Law
of the Town of Southold, have been erected and no alteration
to any of said structures since the above specified dates
have been made.
I am familiar with the above premises by reason of
the fact that I bought the premises in 1961 and knew that the
house had existed in its present location prior to the
enactment of the zoning ordinance in 1957. I lived in
Mattituck all my life and know that the house existed prior
to zoning and I have not made any additions to the house.
This affidavit is made to induce the Town of
Southold, through its appropriate officials, boards or
employees, to issue a letter of pre-existing use; and knowing
and intending that the said Town, its officials, boards or
employees will rely upon and be induced to act by reason of
the representations made herein.
