APPLICATION FOR CERTIFICATE OF OCCUPANCY
Instructions
This application must be filled in typewriter OR ink, and submitte~to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal--(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of pl,,operty showing all property lines, streets, buildings and unusual natural or
topographic featu res.
2. Sworn statement of owner or previous owne~ as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.

MARTIN Th. STEEGE, being duly sworn, deposes and
That he is the owner of premises situated on Dogwood
Lane, East Marion, New York, as is shown on the annexed survey.
That your deponent acquired Lot 88 in 1950, at which
time he built the original structure on Lot No. 88. In 1960 your
deponent acquired part of Lot 87, on which he built an extension to
the existing house.
That your deponent acquired the required permits,
namely No. Z1035, for which he has been issued Certificate of
Occupancy No. Z 702, dated July 9, 1960, and Permit No. 6118Z, for
which he has been issued Certificate of Occupancy No. Z4993, dated
January 31, 1973, for the addition and other improvements made to
the premises. Copies of said Certificates are attached hereto.
That as heretofore stated the original house was
built prior to 1957, and is as shown on the survey. That the house
is in very good condition.
