That I make this affidavit in an effort to secure a Certificate of Occupancy in a pre-existing dwelling, to wit: Premises at (no #) Main Road, Peconic, NY. 3. The aforesaid premises and improvements thereon were constructed approximately forty-six (46) years ago. 4. That upon information and belief, the premises have been used without change, as a two-story private one-family dwelling since the date of its construction. 5.
