I am the Co-executor of the premises situate at (no #) Main Road, Peconic, New York, and as more particularly described in the annexed photocopy of survey of premises dated August 27, 1987. 2. That I make this affidavit in an effort to secure a Certificate of Occupancy in a pre-existing dwelling, to wit: Premises at (no #) Main Road, Peconic, NY. 3. The aforesaid premises and improvements thereon were constructed approximately forty-six (46) years ago. 4. That upon information and belief, the premises have been used without change, as a two-story private one-family dwelling since the date of its construction. 5. That upon information and belief, the premises have not been used for any other purpose than permitted by the Zoning Ordinance. Laura S.
