APPLICATION FOR CERTIFICATE OF OCCUPANCY
A. This application must be filled in by typewriter OF, ink and submitted to the building
inspector with the following: for new building or new use:
1. Final survey of property with accurate location of all buildings, property lines,
streets, and unusual natural or topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal(S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains
less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings
and installations, a certificate of Code Compliance from architect or engineer
responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non -conforming uses, or buildings and
"pre-existing" land uses:
1. Accurate survey of property showing all property lines, streets, building and
unusual natural or topographic features.
2. A properly completed application and a consent to inspect signed by the applicant.
If a Certificate of Occupancy is denied, the Building Inspector shall state the
reasons therefor in writing to the applicant.

HOUSING CODE INSPECTION
September 16, 1977

Upon receipt of an application for a Pre -,Existing Certificate
of Occupancy, I made an inspection of the buildings on the premises.
I was admitted to dwelling by Brian Murphy and began this inspection
at approximately 8:50 A.M.
The dwelling is a wood framed structure constructed on a founda-
tion that was built at different intervals and consists of poured
concrete, rubble stone, red brick masonry, and concrete block.
Cellar is mostly full, east side is crawl space. First floor con-
sists of a living room, front entry hallway, dining room, kitchen,
laundry room, bathroom, enclosed porch, and vestibule. Second floor
has four bedrooms and a full bathroom. Heat is furnished to all
rooms from an oil fired hot water furnace located in cellar. Do-
mestic hot hater is furnished to all plumbing fixtures from an
electric hot water heater. Accessory structures located in the
rear yard consist of a two -car garage with concrete floor, a work
shop on posts with wood floor, a large barn, and five small buildings.
All buildings are wood framed structures.
The following violations of the Housing Code, Chapter 52,
Code of the Town of Southold, New York, were found:
Vestibule - entry, no means of controlling light on entry,
Article V, Section 52-56 B 2.
Ceiling light fixture - porch, not properly mounted, hanging
y conductor for fixture, Article V, Section 52-56 A 1, 2.
Electrical wall switches - require plates, front hall at entry,
partition dividing hallway and living room ('this switch arcs),
laundry room, northwest part of building, Article V, Section
52-56 A 1. '
Lamp cord outlet - bedroom, southwest, on baseboard of south
wall, Article V, Section 52-56 A 1.
Sewage system - west side of cellar, no house trap, Article V,
Section 52-51 C 1.
Cellar stairs - interior, requires hand rail on open side,
Article II, Section 52-27 B.
Cellar entries - from exterior, west side door broken, north
side one door of pair is off its hinges, Article III, Section
52-31 A.

Two -car garage - roof shingles deteriorating causing excessive
openings, electrical service box requires panel cover, three
electrical outlets and two junction boxes require corers, two
broken window panes and one is missing from lower sash of
window on south wall, Article III, Section 52-31 B, C and
Article V, Section 52-56 A 1.
Storage building - entry door off its hinges, window pane
missing from sash on south wall, Article III, Section 52-31 B.
Five small buildings - missing siding, roofing, windows in
a deteriorating state, Article III, Section 52-31 A, B, C.
Inspection completed at approximately 9:30 A.M.
