Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa- tions, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5. Submit Planning Board approval of completed site plan requirements where applicable. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing" land uses: 1. Accurate survey of p~operty showing all property lines, streets, buildings and unusua~ natural or topographic features. 2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings. 3. That he worked for his father in the family building and contracting business in the middle 1960's and further, that he helped construc the improvements on a certain premisis on a private road off Westphalia Avenue, Mattituck, known as Tax Map #1000-113-09-011, f~ Mr. Julius Gimbernat during that time. That he states that both ti main house and the detached cottage/garage were constructed at the same time. This affidavit is made to induce the Town of Southold to issue a seperate Certificate of Occupancy for the cottage/garage.
