tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unulual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.

1) That I am an attorney licensed to practice law in the State of
New York with offices at 828 Front Street, Greenport, New York;
2) That as an associate of Irving L. Price, Jr., Esq. another
attorney licensed to practice law in the State of New York, I have had conver-
sations with Paul Smith, the owner of property designated on the Suffolk County
Tax Map as District 1000 - Section 78 - Block 4 - Lot 32;
3) That said property was purchased by Paul Gustav Sundquist from
William D. Griswold on September 10, 1954. Subsequently the property was
conveyed by Paul Smith as the executor of Paul Gustav Sundquist to said Paul
Smith on September 16, 1981;
4) That throughout the term of ownership of Sundquist and Smith,
used as a private one-family dwelling~/$ /~ ~,~
