Upon request of the Southold Town Building
Department I made inspection of this one story,
two bedroom bungalow and found no violations of
Local Law No. 1, Housing Code of the Town of
Southold.
I picked up key at the Lewis Realtors office
and returned key to Mr. Lewis who arrived as I
was completing my inspection. Inspection began
at approximately 11:00 a.m. and completed at
approximately 11:20 a.m.
This building is of single board and batten
siding, with no insulation in walls or ceilings
and the inside walls and ceilings are unfinished.
With the single floor furnace in livingroom it is
inconceivable that the bedroom and bathroom
temperature be maintained as outlined in Section 513a.

APPLICATION FOR CERTIFICATE OF OCCUPANCY
Instructions
A. This application must be filled in typewriter OR ink, and submitted in triplicate to the Building
Inspector with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and
unusual natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage dispcsal—(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and
installations, a certificate of Code compliance from the Architect or Engineer responsible for
the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non -conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of property showing all property lines, streets, buildings and unusual natural
or topographic features. /
2. Sworn statement of owner or previous ow4r as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent in-
formation required to prepare a certificate.

Ralph J. Kreitzman, being duly sworn, deposes
and says that he is associated with Hughes Hubbard & Reed,
attorneys for Mrs. Emma T. Smith, that Mrs. Smith is the
owner of the above -noted premises, that the building on
said premises was erected sometime before July 6, 19482
that said building is used and has been occupied as a one
family dwelling, is furnished as such and that the build-
ing is in good condition.
This affidavit is made for the sole purpose of
inducing the Town of Southold to issue a Certificate of
Occupancy to Mrs. Smith for said premises and is based
upon affiant's information and belief.
