Smith, Jr. in connection with Application for Certificate of Occupancy on Pre-existing Building on property identified as County Tax Maw No. 1000, Section 24, Block 02, Lot 09, Suffolk County, New York. I, Edgar Jo Smith, Jr., being duly sworn, state that: The buildings on the property consist of a one and one half story frame house with two one story additions and attached decks, and a detached garage. The original buildings and garage were built approximately 1920. The additions have been covered by Certificates of Occupancy No. Z5884 and No. Z13807. The building and garage have been used as a private summer cottage continuously since its beginning. 4. The building and garage are in good condition. Edgar ~/Smith~.
