Instructions
This ~;pplication must be filled in typewriter OR ink, and submitted m ~ to the Building Inspec-
tor with the following; for new buildings or new use:
1, Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features,
2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal),
3. Approval of electrical installation from Board of Fire Underwriters.
4, Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the buUding,
5. Submit Planning Board approval of completed site plan requirements where applicable.
For existing buildings (prior to April 1957), Nonconforming uses, or buildings and "pre-existing"
land uses: ..
1. Accurate survey of pzoperty showing all property lines, streets, buildings and unusual natural or
topographic features.
2.Sworn statement of owner or previous owner as to use, oCCapancy and condition of buildings.

AFFIDAVIT AND STATEMENT by Edgar J. Smith, Jr. in connection
with Application for Certificate of Occupancy on Pre-existing
Building on property identified as County Tax Maw No. 1000,
Section 24, Block 02, Lot 09, Suffolk County, New York.
I, Edgar Jo Smith, Jr., being duly sworn, state that:
The buildings on the property consist of a one and one
half story frame house with two one story additions
and attached decks, and a detached garage.
The original buildings and garage were built
approximately 1920. The additions have been covered
by Certificates of Occupancy No. Z5884 and No. Z13807.
The building and garage have been used as a private
summer cottage continuously since its beginning.
4. The building and garage are in good condition.
