This application must be filled in typewriter OR ink, and suomitted 'v, :~o' ~he Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property dnes, streets, and unusual
natural or topographic featu res.
2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
§. Submit Planning Board approval of completed site plan requirements where applicable.

CHARLES R. CUDDY, being duly sworn, deposes and
That I am the attorney for the Executor of the Estate
of Roberta G. Sinnott. Roberta G. Sinnott died on August 5,
1984. Prior to that time I knew Mrs. Sinnott for more than
15 years. I knew from her and her husband, who predeceased
herr that they had lived in the home at Nassau Point,
Cutchogue, for more than 20 years, and it had been owned by
her family for many years prior to that time.
The records of the Assessor show that the Sinnott
property was improved by a home prior to 19~. I have been
advised that there have been no exterior alterations or
changes to the home since that date.
I respectfully request that the Building Department
of the Town of Southold issue a certificate of occupancy for
pre-existing use for the Sinnott home and property at Nassau
Point, Cutchogue, New York.
