Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic features. 2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa- tions, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5. Submit Planning Board approval of completed site plan requirements where applicable. For existing buildings (prior to April 1957), Non-conforming uses, or buildings ~nd "preexisting" land uses: 1. Accurate survey of property showing all property lines, streets, buildings and unusual natural or topographic features. 2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings. 3. DAUKAS, residing at 120 Boulder Circle, Glastonbury, Connecticut 06033, being duly sworn, deposes and says: That I am a niece of MARTHA B. SHORT, the owner of premises located at (No #) Peconic Bay Blvd., Laurel, New York, Suffolk County Tax Map #1000-128.00-03.00-016.000, as shown on the enclosed survey, who died a resident of Suffolk County on 5/6/88, seized and possessed of the foregoing premises. MARTHA B. SHORT and her sister, MARIE J. BUHSEN purchased the residence in 1950 from William H. Wertenberg, shortly thereafter they constructed the shed in the rear of the property. From the time of purchase of the property, deponent has visited the property at least once a year and states that no further alterations or improvements were made to the premises after April, 1957 that would require a Certificate of Occupancy. I make this affidavit to the Town of Southold for a Certificate of Pre-Existing Use for the residence and shed.
