This application must be filled in typewriter OR ink, and submitted in duplicate to the Building Inspec- tor with the following; for new buildings or new use: 1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic featu res. 2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal), 3. Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa- tions, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5. Submit Planning Board approval of completed site plan requirements where applicable. Premises: 42 H Eugene's Road Cutchogue, New York STATE OF NEW YORK ) SS.: COUNTY OF SUFFOLK ) MARILYN SHEPISH being duly sworn, deposes and says: That I am one of the owners in fee simple of the premises known as 42 H Eugene's Road, Cutchogue, New York and I make this affidavit in support of my application for a certificate of pre-existin~ use. That my husband and I purchased the premises in 1964 and have used the premises continually since that time as a residential, one family dwelling. That the premises has always been used as a residential one-family dwelling and this information is based upon the fact that the house was constructed in 1830 by James Wells Horton and the premises, up until the time it was purchased by my husband and myself, was owned by a member of the Horton Family. Further, the premises is listed with the Historical Society of the Town of Southold and affixed to the side of the dwelling is a plaque .setting forth the fact that the dwelling was built by one James Wells Horton in 1830. That attached hereto is a photograph of the said plaque.
