This application must be filled in typewriter OR ink, and submitte~to the Building Inspec- tor with the following; for new buildings or new use: 1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic features. 2. Final approval of Health Dept. of water supply and sewerage disposal--(S-9 form or equal). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa- tions, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5. Submit Planning Board approval of completed site plan requirements where applicable. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing" land uses: 1. Accurate survey of peoperty showing all property lines, streets, buildings and unusual natural or topographic featu res. 2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings. 3. SHAW, owner of the premises located at (no number) Robinson Road, Southold, New York, which premises is further identified by Suffolk County Tax Map Numbers: 1000~ 081.00~ 01.00~ 024.005. 3) That I have known the owners of this premises, i.e., Laurel A. Shaw and preceeding her~ her father, John H. Schutte, and the premises itself since June, 1958 and that during that time the premises has been used and occupied solely as a one-family dwelling and that such use continues up to and including this date and that the condition of the residence and garage on the premises can be described as good. 4) That I am making this affidavit at the request of the Town of Southold Building Department as part of an application for a "Pre-Existing Certificate of Occupancy" for the premises.
