2. Final Approval from Health Dept. of water supply and sewerage-disposal(S-9
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system cont:

5. Commercial building, industrial building, multiple residences and similar bi

6. Submit Planning Board Approval of completed site plan requirements.

I designed a new beam to replace the rotted beam in the basement of 11600 Main Road in
East Marion. On this date I inspected the completed installation of the new beam and it is
adequate to support the required loading.
Also on this date I inspected the gas fired boiler in the basement. The boiler and
