A. For new building or new use:
1. Final survey of property with accurate location of all buildings,property lines, streets,and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal(S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1%lead.
5. Commercial building,industrial building,multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.

I am a New York State licensed Professional Engineer, retained by Verizon Wireless (VZ) in connection
with the captioned matter above. I have reviewed the completed VZ antenna and associated
equipment installation at the existing wireless telecommunications facility. It is my opinion that the
completed installation substantially complies with the project plans and specifications, dated January 7,
2016 (REV 2 FOR FILING) as permitted except as listed below.

d.Upon approval of this application,the Building Inspector will issue a Building Permit to the applicant. Such a permit

issues a Certificate of Occupancy.
f. Every building permit shall expire if the work authorized has not commenced within 12 months after the date of
issuance or has not been completed within 18 months from such date.If no zoning amendments or other regulations affecting the
property have been enacted in the interim,the Building Inspector may authorize, in writing,the extension of the permit for an
addition six months.Thereafter, a new permit shall be required.

As an addendum to the attached SPUD, the Planning Department has conducted a
review of the proposed modifications pursuant to §280-74 B.(2), and has received a
report from our Wireless Technical Consultant, Cityscape, Inc. (see attached report).
We have the following recommendations for this application:
1. The proposed Verizon modifications and up-grades are in compliance with the
General Requirements of§280-70, therefore, we recommend a Building Permit
be issued for this application in accordance with the Structural Analysis Report,
dated December 14, 2015, and the FCC RF Compliance Assessment and
Report, dated January 7, 2016;
2. The facility shall remain secured and protected from unauthorized personnel.

At your request, on behalf of the Town of Southold("Town"),CityScape Consultants,Inc.
("CityScape") in its capacity as telecommunications consultant for the Town, has considered the
merits of the above referenced application submitted by Verizon Wireless("Applicant")to modify
its equipment on an existing one hundred thirty-one foot six inch(131'6")lattice tower. The tower
is located at 165 Peconic Lane, Peconic,New York,see Figure 1.
Support Structure &Equipment
The existing support structure is depicted in Appendix, Exhibit A. The Applicant currently
has nine (9) antennas (3 per sector) at the one hundred (100) foot level. It is proposed to replace
all nine (9) antennas, add nine (9) RRUs and three (3) distribution boxes on the tower. Six (6) of
the existing sixteen (16) coaxial feed lines will remain and three (3) hybrid fiber feed cables will
be added. The proposed antenna layout is depicted in Appendix, Exhibit B. No changes will be
made at ground level outside of the existing shelter, with the exception of adding a second GPS
antenna.
The referenced facility and tower has been designed, constructed and continues to be
operated by the tower owner and professionals with expertise in telecommunications site design
and construction. The facility has been designed to maximize the use of telecommunications
facility and will have the capability to provide space for future collocation opportunities.

Structural Analysis
The Tower Owner has submitted a structural analysis, dated December 14, 2015 based on
ANSI/EIA/TIA-222-F standards. This analysis indicates that the tower will be loaded to 88% and
the foundation is listed as adequate,,see Appendix, Exhibit C.
The Applicant submitted a complete study indicating compliance with the FCC's Radio
Frequency(RF) energy and exposure limits,see Appendix, Exhibit D.
Under the Middle Class Tax Relief and Job Creation Act of 2012 ("The Act")any personal
wireless facility collocation, modification or upgrade may qualify for streamlined processing.
Furthermore,under Section 6409(a)it can qualify for administrative approval if the request meets
six criteria,most notably(1)it does not increase the structure height by 10% or 20 feet,whichever
is greater, (2) it does not increase the structure width by 20 feet and (3) it does not require any
excavation outside the existing ground compound or leased area. Section 6409(a) further states
that if an application meets the criteria, the application should be approved and not denied.
Cityscape has determined this application complies with the Act and recommends approval, with
the following condition:
The facility shall remain secured and protected from unauthorized personnel.

STRUCTURAL AiYALYSIS:
Methodology:
The structural analysis was done in accordance with the New York State Building Code and
TIA/EIA-222, Revision F (TIA), Structural Standards for Steel Antenna Towers and Antenna
SM12orting Structures, and the American Institute of Steel Construction (AISC), Manual of
Steel Construction,Allowable Stress Design,Ninth Edition.
The analysis was conducted using a fastest mile wind speed of 85 mph(equivalent to 100 mph
3-second gust) and 3/a" of radial ice over the structure and associated appurtenances. The TIA
Standard requires a basic wind speed of 85 miles per hour for Suffolk'County,New York.
Two loading conditions were evaluated in accordance with TIA/EIA-222-F to determine tower
capacity.The more demanding of the two cases is used to calculate tower capacity:
• Case 1 =Wind Load(without ice)+Tower Dead Load
• Case 2=0.75 Wind Load(with ice)+Ice Load+Tower Dead Load
The TIA/EIA standard permits a one-third increase in allowable stresses for towers less than
700-feet tall. Allowable stresses of tower members were increased by one-third when
computing the tower capacity values shown below.
Analysis Results:
Analysis of the tower was conducted in accordance with the criteria outlined herein with
antenna changes as previously described. The following table summarizes the results of the
analysis based on stresses of individual leg and bracing members:

The results of the analyses provide a clear demonstration of compliance with the
FCC general population MPE limit. Moreover, because 'of the conservative
calculation methodology and operational assumptions applied in the analysis, the
RF levels actually caused by the combination of antennas at the site will be less
significant than the calculations indicate.
