framed dwelling with attached garage and two decks; shed, a bluestone
driveway; bulkhead. All in 'A' -Residential Agricultural zone with access
to Mason Drive; a Town maintained road.

APPLICATION FOR CERTIFICATE OF OCCUPANCY`-
Instructions
A. This application must be filled in typewriter OR ink, and submitted n a� to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal—(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non -conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of property showing all property lines, streets, buildings and unusual natural or
topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.

sworn, deposes and says:
On August 25, 1952, we purchased property at 1650 Mason Drive
Cutchogue, New York, from William H. Mason and Harry E. Mason by
deed dated August 25, 1952, and recorded in the Suffolk County
Clerk's Office on August 27, 1952, in Liber 3398 Page 462 (Suffolk
County Tax Map No. District 1000, Section 104.00, Block 07.00,
Lot 011.000).
In 1953 we had erected on our property a small one-story
frame house. After hurricane Carol in 1954 our house was raised
approximately 25 inches and in 1955 we added a wood deck, 16' x 10
on what is now the westerly rear portion of the house so we would
have access to the house from the rear yard.
On July 26, 1963, we applied for a building permit, No. 2128
from the Building Department of the Town of Southold to build an
addition on the easterly portion of the dwelling. On October 7,
1963, we received a Certificate of Occupancy from the Building
Department, No. Z1705.
During the summer of 1974 we added an 8' x 10' deck in the
rear yard area of the easterly wing of the house. At the time we
were unaware that a small free standing deck had to have a buildin<
permit from the Building Department. This affidavit is made to
acquaint the Building Department of the Town of Southold with the
facts surrounding the dwelling at 1650 Mason Drive, Cutchogue, New
York, as portions of the building were erected before zoning and
to obtain a present Certificate of Occupancy for the dwelling and
in particular the small deck located in the rear portion of the
easterly side of this building.

1. In 1952 we purchased the property located at 1650 Mason
Drive, Cutchogue, New York, with the Suffolk County Tax Map
esignation of: District 1000, Section 104, Block 7, Lot 11.
2. In 1953 we erected a one-story frame house on this
roperty and in 1955 we erected a deck on the rear of the house
hich is approximately 17 ft. x 10 ft. and which is shown on the
attached survey by Roderick Van Tuyl, P.C. dated August 21, 1986
the westerly two decks on the rear of the house.
3. This affidavit is made to obtain a pre-existing
icate of Occupancy for this westerly deck from the Southold
Building Departmen
