Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic feature[ 2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa- tions, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5. Submit Planning Board approval of completed site plan requirements where applicable. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing" land uses: 1. Accurate survey of peoperty showing all property lines, streets, buildings and unusual natural or topographic features. 2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings. 3. DRUM, being duly sworn, deposes and says: THAT he resides at (No#) Mill Road, Mattituck, New York, and that he has been a resident of Southold Town for the past ~years; THAT he is familiar with the property known as 4230 Grand Avenue, Mattituck, N.Y., shown and designated as Lot #1 on "Minor Subdivision Plan for Lynn San Andres", prepared by Young & Young, Licensed Land Surveyors, Riverhead, N.Y., dated 1/5/83 and last amended 4/23/84, which map was approved by the Southold Town Planning Board on 7/27/84. A copy of that portion of the above mentioned map, showing Lot #1, is attached hereto. THAT on personal knowledge, information and belief, your deponent can state that the structures shown on said survey were built prior to April 23, 1957. That a Certificate of Occupancy (pre-existing) No. Z10836, was issued by the Building Inspector of the Town of Southold on December 30, 1981, for the subject property prior to its subdivision. That there have been no changes to the present existing structures on the property, other than normal main- tenance, since prior to April 23, 1957. THAT this property (Lot #1) has access on a public road, and that this application is made for an update of the existing C.O. to reflect the property and the structures thereon as they presently exist.
