Instructions
A. This application must be filled in typewriter OR ink, and submitted in DUPLICATE to the Building
Inspector with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and
unusual natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal—(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and
installations, a certificate of Code compliance from the Architect or Engineer responsible for
the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.

Upon receipt of an application for a Pre -Existing Certificate
of Occupancy I made an inspection of this one story framed
dwelling. I was admitted to building by Mr. Denis Salzmann
and began this inspection at approximately 2:10 P.M.
The building consists of three bedrooms, kitchenette,
bathroom, living room, enclosed porch and an attached one
car garage. Construction is light framing, vertical siding,
shell only. Heat is furnished to building from an oil fired
warm air floor furnace. Foundation is cement block with
crawl space and partial cellar. A utility room on the east
side of garage contains water pump and gas fired hot water
heater.
The following violations of The Housing Code, Chapter 52,
Town of Southold, were found:
Kitchenette: Gas cooking range, no shutoff valve
at unit. Article V. Sec. 52-52 B.2.
Fireplace Inner hearth disintegrating (no fire
brick), entire poured cement structure
shattered through to rear, (Bedroom
side) - wood trim above fire place
opening charred. Article V, Sec. 52-41
B -C.
Space Heater: Free standing in living room - gas fired,
no shut-off valve at unit - vent pipe not
securely connected to unit. Vent pipe
not insulated (properly) through opening
of building's wood siding. Article V,
Section 52-52 B-2, Sec. 52-54 A.1.2. & B.
