Instructions
A. This application must be filled in typewriter OR ink, and submitted-~to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic featu res.
2. Final approval of Health Dept. of water supply and sewerage disposal--(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5.Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of peoperty showing all property lines, streets, buildings and unusual natural or
topographic featu res.
2.Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.

STANLEY RYSKO, being duly sworn, deposes and says:
That he and his wife are the owner of premises at Main Road,
Cutchogue, New York (Suffolk County Tax Map No. District 1000,
Section 097.00, Block 01.00, Lot 013.000). That they acquired
title to this parcel by deed dated December 19, 1949 and which was
recorded in the Suffolk County Clerk's Office on December 20,
[949 in Liber 3028 Page 580 and by deed dated April 21, 1950
and recorded in the Suffolk County Clerk's Office on April 24,
1950 in Liber 3068 Page 176. The premises was previously owned
by my mother, Sophie Gryszko since 1945 and has been in the
~amily since that time.
Located on the property is a one-story frame dwelling in
'ood condition and a detached garage also in good condition.
The application is made for the purpose of obtaining a
~re-existing Certificate of Occupancy from the Building Department
Df the Town of Southold.
