This application must be filled in typewriter OR ink, and submitted in duplicate to the Building Inspec- tor with the following; for new buildings or new use: 1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic features` · 2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa- tions, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5. Submit Planning Board approval of completed site plan requirements where applicable. B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing" land uses: 1. Accurate survey of p~operty showing all property lines, streets, buildings and unusual natural or topographic features. 2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings. 3. Rowland, located at (No #) Main Road, Cutchogue, New York, Suffolk County Tax Map #1000-109.00- 01.00-023.000, since they are close to my farm where I was born and brought up. The residence is a 19th Century structure and was erected long before 1900. The shop may not be of the same vintage, but it certainly was constructed long before 1957 and this applies also to the garage and sheds. A copy of the survey is attached hereto. To my knowledge, there have been no changes or alterations made to the buildings that would require a Building Permit or Certificate of Occupancy, since 1957. I make this application to the Town of Southold for a Certificate of Pre-Existing Use for the two-story frame dwelling, two-story shop, sheds and garage.
