This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1%lead.
5. Commercial building, industrial building,multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.

The Southold Town Board of Trustees reviewed the survey dated July 12, 1995, and after an
inspection made on February 13, 2002, determined the construction of a second-story addition to
the existing dwelling, to be out of the Wetland jurisdiction under Chapter 97 of the Town
Wetland Code.
However, any activity within 100' of the Wetland line would require permits from this office.
This determination is not a determination from any other agency.

Dear Mr. Sullivan:
A review has been made of your proposal to:
Construct a 2nd floor addition to an existing single family dwelling, with no expansion of
the footprint of the structure. The proposed addition is greater than 75' from the tidal
wetland boundary. This determination is based upon the plan prepared by Frederick R.
Weber on January 18, 2002.
Based on the information you have submitted, the New York State Department of
Environmental Conservation has determined that the project is listed in 6NYCRR Part 661.5
(Tidal Wetlands Land Use Regulations) as a use not requiring a permit. Therefore, no permit
is required under the Tidal Wetlands Act (Article 25 of the Environmental Conservation Law).
Please note that any additional work, or modification to the project as described, may require
authorization by this Department. Please contact this office if such are contemplated.
Please be further advised that this letter does not relieve"You of the responsibility of obtaining
any necessary permits or approvals from other agencies.
