Upon receipt of an application for a Pre -Existing Certificate of
Occupancy I made an inspection of the buildings on this premises. I
picked up key from the Jack Driscoll Realty office, C.R. 27, Mattituck,
N.Y. and began this inspection at approximately 10:30 A.M.
The dwelling is a wood framed one story structure constructed on
cement block piers and contains four bedrooms, living room with dining
area, kitchen, two bathrooms and a screen enclosed rear porch. A
wood framed accessory storage building is located in the rear yard
area. This building is of new construction and iff in violation of
Zoning Ordinance, Chapter 100, Article XIV, Section 100-141 in that no
building permit has been issued or applied for.
The following violations of the Housing Code, Chapter 52, Code of
the Town of Southold, N.Y. were found:
Sewage drainage system -no fresh air vent. Article V. Section
Itchen sinks, no water seal traps. Article V, Section
52-51 C.5.
EhtrY-front to living room, no means of controlling light in room
at point of entry. Article V, Section 52-56 B.2.
Safety devices -two electric hotwater heaters, no safety pressure
relief va ves. Article V. Section 52-53 I.1.
Electrical-hotwater heaters -plates on shut off switches for
e ec ric hotwater heaters, no clamp for ground wires loose
terminal block on unit in south end of house, also improper
wiring. Article V, Section 52-56 A-1.2.
Inspection completed at approximately 11:15 A.M. and key was
returned to Jack Driscoll. i)

Instructions
A. This application must be filled in typewriter OR ink, and submitted in DUPLICATE to the Building
Inspector with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and
unusual natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal—(5-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and
installations, a certificate of Code compliance from the Architect or Engineer responsible for
the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non -conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of property showing all property lines, streets, buildings and unusual natural
or topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent in-
formation required to prepare a certificate.

that she resides at 32 Chelsea Drive, Syosset, New York and is
over the age of 21 years.
1. I along with my husband, JOSEPH V. ROCCARO as tenants
by the entirety own in fee all that certain, plot,piece or parcel
of land with the buildings and improvements thereon erected,
situate, lying and being at Peconic, Town of Southold, County of
Suffolk and State of New York, shown and designated as Lots 61
and 62 on a certain map entitled "Map #2 of Peconic Shores, Peconic
Long Island, New York property of B.B. Bailey and C.H. Bailey,
surveyed, May 21, 1929 by Daniel R. Young, P.E. and L.S." filed
September 15, 1930 as map #65
2. That my husband and I acquired title to said premises
from Andrew Carril and Adolph Carril by deed dated January 8, 1974
and recorded in the Office of the Clerk of the County of Suffolk
on January 14, 1974 in Liber 7569 page 147.
3.That since my husband and I acquired possession of
said premises in January of 1974 they have been used solely and
exclusively as a summer residential one family dwelling.
