Instructions
A. This application must be filled in typewriter OR ink, and submitted in duplicate to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey ot property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final aporoval of Health De~t. of water supplv and sewerage disposal-(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of peoperty showing ali property lines, streets, buildings and unusual natural or
topo§raohic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildi.ngs or premises, or other pertinent informa-
tion required to prepare a certificate.

EDNA A. ROBB, residing at Briarwood Lane, Cutchogue, New
York, being duly sworn, deposes and says:
1. That she is the owner of the premises on the South
side of Briarwood Lane, Cutchogue, New York, which is being con-
veyed to Fred J. Car¢ich and Mary Ann Smigelski.
2. That on said premises is a garage on the South side
of the residence.
3. That the premises were purchased from William F.
Walsh and wife in February of 1951.
4. That the garage was on the premises at that time,
and deponent believes that it was constructed in about 1933.
5. That no changes have been made in the garage since
it was purchased by deponent and her husband in 1951.
