JOANNE RITTER, being duly sworn, deposes and says:
I reside at 2585 Peconic Lane, Peconic, New York, 11958.
The premises shown on the annexed survey made by RODERICK VAN
TUYL, P.C., dated June 9, 1983, and redated June 11, 1985, and made a
part of this affidavit by reference, accurately represent the premises
as they existed prior to April, 1957.
No other structures as defined by the Zoning Law of the Town
of Southold have been erected and no alteration to any of said struc-
tures since the above specified dates have been made.
I am familiar with the above premises by reason of the fact
that I am one of the fee title owners of the premises.
This affidavit is made to induce the Town of Southold, through
its appropriate officials, boards, or employees, to issue a letter of
pre-existing use; and knowing and intending that the said Town, its
officials, boards or employees will rely upon and be induced to act by
reason of the representations made herein.

Instructions
A. This application must be filled in typewriter OR ink, and submitted m ~mmmlm to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2.Final approval of Health Dept. of water supply and sewerage disposel-(S-9 form or equal).
3.Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5.Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non-conforming uses,' or buildings and "pre-existing"
1. Accurate survey of pz"operty showing all property lines, streets, buildings and unusual natural or
topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.
