APPLICATION FOR CERTIFICATE OF OCCUPANCY
Instructions
A. This application must be filled in typewriter OR ink, and submitted in DUPLICATE to the Building
Inspector with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and
unusual natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal—(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and
installations, a certificate of Code compliance from the Architect or Engineer responsible for
the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non -conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of property showing all property lines, streets, buildings and unusual natural
or topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent in-
formation required to prepare a certificate.
C. Fees:

Housing Code Inspection
March 29, 1978
1000 Minnehaha Blvd.
Southold, N.Y.

Upon request of the office of Lefferts Edson, Attorney, Main
Road, Southold, N.Y. I made an inspection of the buildingson this
premises. I was admitted to dwelling by Mr. Rios who accompanied
me during this inspection that began at approximately 1:40 P.M.
The dwelling is a one and one half story wood framed building
constructed on a cement block foundation. First floor contains a
living room, two bedrooms, kitchen and a full bathroom. Second
floor has two bedrooms. Heat is furnished to all rooms from an oil
fired hotwater furnace located in full cellar. Cellar also contains a
half bathroom, shower room and kitchen facilities, i.e. kitchen
sink and electric cooking range. A detached wood frame one and one
half garage is located in the rear yard area.
A family room addition is presently under construction on north
side of dwelling. Building Permit #95432 dated November 7, 1977.
The following violations of the Housing Code, Chapter 52, Code
of the Town of Southold, N.Y. were found:
Kitchen -gas cooking range, no shut off valve at unit.
Article V. Section 52-52 B.2.
Sewage System -no house trap. Article V, Section 52-51 C.1.
Shower room in cellar -no ventilation provided. Article V,
Section 52-26 B. a.b.
Plumbing fixtures -in cellar waste lines not tied into buildings
sewage system. Article V, Section 52-51 C.1.
Inspection completed at approximately 2:05 P.M.
