This application must be filled in typewriter OR ink, and submitted in duplicate to the Building Inspector with the following; for new buildings or new use: 1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic features. 2. Final approval of Health Dept. of water supply and sewerage disposal—(S-9 form or equal). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installations, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5. Submit Planning Board approval of completed site plan requirements where applicable. B. For existing buildings (prior to April 1957), Non -conforming uses, or buildings and "pre-existing" land uses: 1. Accurate survey of property showing all property lines, streets, buildings and unusual natural or topographic features. 2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings. 3. Rich & ano Helen McCafferey-Leasee Upon request of the Southold Town Building Department I made inspection of this two story framed building and found the following violations of Local Law #1, Housing Code Town of Southold. This is a commercial building with the principal operation being a grocery store with a snack bar. Heat is supplied to the store only with the furnace located in a partial cellar. My inspection began here at approximately 11:15 a.m. Cellar: Hot water heater, not connected to flue - Se tc ion 520a; ground wire to well point pipe not clamped securely - Section 528a, e; electric hook up to water pump motor, no ground connection (line has no ground wire), no connector on motor junction box and this line to fuse box (switch type) enters box at door opening, door therefore cannot be closed, fuse box is deteriorating = Section 528a, b, c, e, f; Fuse box (4 circuit) near hot water heater, door open due to electric wires not being nested in box, trouble lamp type cord (2 wire) connected to circuit #3, termination not traced - Section 528a, c, e, f; furnace, no cover for stack switch - Section 528a, f; Compressor for refrigerator in store, connector not secured to junction box on unit - Section 528a, b.; fuel line to furnace on cellar floor, not protected Section 107; cellar light, pull chain type fixture, no light switch at point of entry to control light - Section 529b; no pul]con fixture - Section 528a. Store: Open junction box in ceiling, northeast end - Section 528a. Kitchen behind store: Water line from sink ind6xed to 4" cast iron sewage line with no sealing of joint - Section 502b. Rich page 2 Rear of building is of barn type construction, siding (vertical) is not tight, roof in need of repair, kitchen and two half baths are in this section of building and are subject to elements and destructive insects - Section 302 b, c and Section 301c. Access to second floor stairway in this section - second floor front and rear section'of building used as storage area for store, under present conditions not suitable for habitation. Three broken window panes on second floor - Section 302b. Exterior: Opening on southwest side corner of store at foundation - Section 302a. Several shingles broken and missing - Section 302b.
