Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic features. 2. Final Approval from Health Dept. of water supply and sewerage-disposal(S-9 form). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1% lead. 5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate of Code Compliance from architect or engineer responsible for the building. 6. Submit Planning Board Approval of completed site plan requirements. For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and "pre-existing" land uses: 'i. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic features. .2. A properly completed application and a consent to inspect signed by the applicant. SMITH, being duly sworn says: I live on Mechanic Street, Southold, New York and have been a builder in that area for more than 40 years. On November 11, 1968 Frederic P. Rich acquired title to premises known as 300 Boisseau Avenue, Southold, on which was located a two-story house and garage, which house had been con- structed more than 50 years before. Shortly after acquiring title, the said Frederic P. Rich engaged my firm to make inter- ior repairs and alterations necessary to improve the interior and make it more suitable for habitation and to divide the in- terior space into two separate apartments with modern kitchen appliances and modernization of bathroom facilities. These in- terior alterations were made in the fall and winter of 1968 and the early spring of 1969. The alterations were completed in the early spring of 1969 and final bill rendered on June 12, 1969, which was paid shortly thereafter. Sworn to before me this~day of July, 1981 STATE OF NEW YORK) ) COUNTY OF SUFFOLK) SS: FREDERIC P. RICH, residing at 52965 Main Road, Southold, N. Y. 11971, submits this affidavit in support of application for certificate of occupancy on house located on parcel shown as Parcel B on the annexed survey map known by street number 300 Boisseau Avenue, Southold, New York on ground of "pre- existing"land use. Said house (and garage) was constructed and used many years prior to April, 1957 as a private dwelling, said house being more than 60 years old. The property was purchased by me on November 11, 1968 and the interior of the two story house renovated, modernized and altered by Daniel T. Smith shortly thereafter in such a way as to make it suitable for two family occupancy, all in the manner set forth in his annexed affidavit. Ever since that time the house has been rented to two families for private occupancy. I am informed that at the time of said renovation, since it did not entail any structural changes to the exterior no build- ing permit was required and no certificate of occupancy was then issued. As the present owner of the house I now make application for a certificate of occupancy on the ground of "pre-existing" land use.
