Instructions
A. This application must be filled in typewriter OR ink, and submitted-~to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal}.
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.

VINCENT RICE, residing at (No #) North Oakwood Road, Laurel, New
York, being duly sworn deposes and says:
Deponent and his wife, DORIS RICE, are the owners of certain property
on the East side of North Oakwood Road, Laurel, New York, which was purchased
in 1946. They immediately thereafter constructed a residence on the premises.
In 1953 they made an addition to the premises consisting of a garage and
two bedrooms and deponent constructed a concrete block patio on the South
side.
Deponent further states that there have been no further additions
to said residence and makes this application to the Building Department of
the Town of Southold to obtain a Certificate of Pre-Existing Use.
Attached hereto is a print of survey made of the property dated
July 28, 1982 by Roderick Van Tuyl, P.C.
