Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic features. 2. Final approval of Health Dept. of water supply and sewerage disposal--(S-9 form or equal). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa- tions, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5. Submit Planning Board approval of completed site plan requirements where applicable. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing" land uses: 1. Accurate survey of peoperty showing all property lines, streets, buildings and unusua~ natural or topographic features. 2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings. 3. STATE OF NEW YORK) SS.: COUNTY OF SUFFOLK) WILLIAM WICKHAM, residing at 115 Old Harbor Road, New Suffolk, New York, being duly sworn, deposes and says that he is an attorney and a member of the firm of Wickham, Wickham & Bressler, P.C. with offices at Main Road, Mattituck, New York; that he is the attorney for HERBERT H. REEVE. HERBERT H. REEVE is the owner of premises located at (No #) New Suffolk Avenue, Mattituck, Town of Southold, County of Suffolk and State of New York; he acquired this parcel from is father HALSEY H. REEVE in 1921 and built the house shortly thereafter. No improvements have been made to the residence since 1957. Survey of said property is attached. The garage which was owned jointly with his brother ELLWOOD REEVE, who acquired the adjoining parcel from HALSEY H. REEVE, his father at the same time. The garage was also built in the 1920's and no improvements have been made on it. Deponent makes this application to the Town of Southold Building Department to issue a Certificate of Pre-Existing Use for the residence and garage. - ( Sworn to before me this ~ day of January, 1986.
