Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic features. 2. Final Approval from Health Dept. of water supply and sewerage-disposal(S-9 form). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1% lead. 5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate of Code Compliance.from architect or engineer responsible for the building. 6. Submit Planning Board Approval of completed site plan requirements. For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and "pre-existing" land uses: l. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic features. 2. A properly completed application and a consent to inspect signed by the applicant. If a Certificate of Occupancy is denied, the Building Inspector shall state the reasons therefor in writing to the applicant. Fees 1. TOOKER, being duly sworn, deposes and says: That he resides at 3 Waterview Court, Riverhead, New York. That he is the Executor of the Estate of Elberta H. Reeve, deceased. That Mrs. Reeve was the owner of premises situated at Park Avenue, Mattituck, New York. That said premises were acquired by Joseph Bunce Hudson, the father of Elberta H. Reeve on September 8, 1919, and were devised to Mrs. Reeve upon the death of her father. That since 1950 there have been no structural alterations or changes to the premises or their use. That your deponent forwarded to the Building Inspector of the Town of Southold on February 8, 1983, a survey of the premises prepared by Young & Young, dated the 14th day of January, 1983. This Affidavit is made to induce the Town of $outhold, through its appropriate officials, boards or employees, to issue a certifi- cate of pre-existing use, knowing and intending that the said Town, its officials, boards or employees will rely upon and be induced to act by reason of the representations made herein.
