NOTICE IS HEREBY GIVEN that the owner of the above premises HAS
NOT CONSENTED TO AN INSPECTION of the premises by the Building Inspec-
tor to determine if the premises comply with all applicable codes and ordin-
ances, other than the Building Zone Code, and therefore, no such inspection
has been conducted. This Certificate, therefore, does not, and is not intended
to certify that the premises comply with all other applicable codes and regula-
tions.

New York, being duly sworn deposes and says:
That deponent is an attorney and a mmmber of the firm of Wickham,
Wickham & Bressler, P.C. with offices at Mm~n Boad, Mattituck, New York and
is the attorney for Doris D. ~=ex~, residing at Boute 48, Mattituck, New York.
Deponent further says that on December 16, 1983 Doris D. Reeve sold
to Claire Uliano property at Mattituck on the corner of B-=eve Avenue and Harbor
View Avenue as shown on the attached survey made for the purchaser. The seller,
however, was to furnish a Certificat~ of Pre-existing Use, but could not de so
until the survey was delivered on the date of closing.
Mrs. B-=eve has informed deponent that the residence was built around
the year 1860 and that it was partially rebuilt after the hurricane of 1938;
that the garag~ was built in the 1930's and that no ~,L~roven~nts or alterations
have been made since 1957.
Deponent makes this Affidavit to induce the Building Depertmant of the
Town of Southold to issue a Certificate of Pre-existing Use for both the residence
