Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic features, 2. Final approval of Health Dept. of water supply and sewerage disposal--(S-9 form or equal). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar bu-ildings and installa- tions, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5. Submit Planning Board approval of completed site plan requirements where applicable. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing" land uses: 1. Accurate survey of peoperty showing all property lines, streets, buildings and unusual natural or topographic features. 2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings. 3. RAFFORD, d~ceased, and the grandson of Edith G. Rafford. Deponent further says that the ma~n house was built in 1955 and 1956 and that the oottage was built prior to 1952. This information was secured from neighbors who have lived on the block and also from a family photo album, showing construction and dates. Attached hereto is copy of a survey of the premises prepared by Lee Lut_z/Lioensed Land Surveyor showing the location of the two houses. Deponent makes this application to the Building Department of the Town of SQuthold requesting a Certificate of Pre-Existing Use for both the main house land the cottage.
