Upon request of Gary F. Olsen, Attorney, Main Road, Mattituck,
N.Y. for a Pre -Existing Certificate of Occupancy I made an inspection
of the buildings at the above captioned premises. I was admitted to
dwelling by Mrs. Purves and began this inspection at approximately
10:45 A.M.
The dwelling is a one and one half story wood frame structure
constructed on a cement block foundation. A two car garage is attached
to dwelling by an open breezeway. Garage is constructed on a cement
block foundation. The first floor of dwelling contains a frdnt entry
hallway, a living room with fireplace, two bedrooms, two full bathrooms,
a kitchen and a laundry room. Second floor contains a large study with
two bedrooms and a full bathroom. Heat is furnished to all rooms on
first and second floor from an oil fired forced warm air furnace located
in partial full cellar. Domestic hotwater is furnished to all plumbing
fixtures within the dwelling from an oil fired hotwater heater located
in the cellar. Access to cellar within dwelling is provided by a stair-
way off the laundry room,the cellar is also accessible from the exterior
by means of doorway located in the foundation wall, on the rear of
building. There is a partial cellar in the foundation under the garage
with an exterior doorway for access. A set of wooden steps from the
Ma;lftg-rear yard -ascends to the open patio:of breezeway.
The following violations of the Housing Code, Chapter 52, Code
of the Town of Southold were found:
Entries -to dwelling - no means of controlling light at point
of entry. Article V. Section 52-56 B 2.
Interior Protection - second floor bathroom - no moisture
resistant floor covering over wood floor. Article V, Section
52-26 A 4.
Removal of products of combustion_- improper smoke stack connection
to oil fired hotwater heater. Article V, Section 52-53 H 1
Sewage system - no house trap provided. Article V, Section 52-
51 C 1.

steps and top landing are not level, from apparent.
deterioration of posts supporting the stair unit.

APPLICATION FOR CERTIFICATE OF OCCUPANCY
Instructions
A. This application must be filled in typewriter OR ink, and submitted in DUPLICATE to the Building
Inspector with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and
unusual natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal—(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and
installations, a certificate of Code compliance from the Architect or Engineer responsible for
the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non -conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of property showing all property lines, streets, buildings and unusual natural
or topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3 Date of any housing code or safety inspection of buildings or premises, or other pertinent in-
formation required to prepare a certificate.
C. Fees:
