of Pre-Existing Use on a frame house and a frame cottage owned
by Deponents on the North side of Meday Avenue, Mattituck, being
lots numbered 58, 59 and 60 on "Map of Tollewood ;

tor with the following; for new buildings or new use:
1. Finai survey of property with accurate location of all buildings, property lines, streets, and unusual
natura~ or topograohic feature~.
2. Finm approval of Health Dept. of'water supply aha sewerage disoosal--(S-9 form or equal).
3. ADorovai of eiectricai installation from Board of Fire unaerwriters.
4. Commercial buildings, industrial buildings, Multiole Residences and similar buildings and installa-
tions, a certificate of Coae comoiiance from the Architect or Engineer responsible for the building.
5. Submit Pianning Boara approval of completed site Dian reauirements where applicable.
For existing buildings (prior to Aoril 1957), Non-copforming uses, or buildings and "pre-existing"
