This application must be filled in typewriter OR ink, and submitted in triplicate to the Building Inspector with the following; for new buildings or new use: 1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic features. 2. Final approval of Health Dept. of water supply and sewerage dispcsal—(S-9 form or equal). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installations, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5. Submit Planning Board approval of completed site plan requirements where applicable. B. For existing buildings (prior to April 1957), Non -conforming uses, or buildings and "pre-existing" land uses: 1. Accurate survey of property showing all property lines, streets, buildings and unusual natural or topographic features. 2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings. 3. Date of any housing code or safety inspection of buildings or premises, or other pertinent in- formation required to prepare a certificate. C. Fees: 1. Certificate of occupancy $5.00 2. OUmlty Commission Er.pjes Marek 30, 19 /�4 HOUSING CODE INSPECTION November 8, 1972 12680 Main Road East Marion, New York C Tax Roll: Joseph Gio4ia and others Unoccupied Upon request of the Southold Town Building Department I made inspection of this two story, four bedroom framed dwelling and found the following violations of Local Law #1, Housing Code of the Town of Southold. I began inspection at approximately 8:50 a.m.' by being admitted to the rear, enclosed porch entrance by Mr. Ruben Wecker. Rear orch: Light socket with lamp cord supported by hook in ceiling - Section 528a. Surface mounted square box electric outlet mounted on west wall with drop cord line running to kitchen outlet through inside window - drop cord to outside light socket through corner of building plugged into this outlet - Section 528a. Living Room: Lamp cord through wall at front entrance for porch light - Section 528a. Enclosed porch: west side, picture window pane broken - Section 302b. Cellar: Access under stairway to second floor through trap door in floor, narrow stairway without hand rail - Section 216a, b. South end of cellar, outlet and ceiling light fixture connected to lamp cord plugged into wall mounted outlet - Section 528a. Stairway to second floor: Loose plaster on walls - Section 303c. Bedroom, second floor, northeast: surface mounted base- board outlet, lamp cord hook up - Section 528a. Exterior of building, trim, facia and eaves, paint peeling - Section 302b.
