This application must be filled in typewriter OR ink, and submitted in duplicate to the Building Inspec- tor with the following; for new buildings or new use: 1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic featu res. 2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa- tions, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5. Submit Planning Board approval of completed site plan requirements where applicable. ~B. STATE OF NEW YORK) COUNTY OF SUFFOLK) SS.: LYDIA NORRIS PELL, being duly swo~'n, deposes and says: That she is the owner of dwelling located in the Town of Southold, on the N.W. side of Dogwood Lane - 5~.67 ft. S.W. of intersection of N.W. side of Dogwood Lane with the N.E. side of Dogwood Lane, which is designated and shown on the Suffolk County Tax Map as District 1000, Section 05~, Block 05 and Lot 056. Deponent has caused to be filed an application to the Southold Town Building Inspector's Office fop a Pre-Existing Certificate of Occupancy of said dwelling. This Affidavit is made to certify that deponent personally knows of her own knowledge that the dwelling was e~eeted in 195 2 and that deponent togethem with her husband, John Norris, resided in said dwelling until the death of John Norris on November 9, 1959. There- after deponent continued to reside in said dwelling until 1976 when she married Mr. Pell and resided with him in G~eenport, Long Island. The dwelling has thereafter continuously been used a a private ~esidenee by rentees to date. Said dwelling has continuously been used as a private home and that to deponent~s knowledge, there has been no changes made in the building of any kind, either prior to or subsequent to the date of the zoning ordinance in 1957. I make this Affidavit knowing that the Building Inspector's Office at the Town of Southold relies upon the truth of the statements herein contained.
