WILLIAM WICKHAM, residing at 115 Old Harbor Road, New Suffolk, New
York, being duly sworn deposes and says that he is the attorney for Marie
Parrsh, owner and seller of premises on the North side of Sound Avenue,
Mattituck, New York adjacent on the West to the Penny Lumber Yard, as shown
on the survey attached.
Deponent is familiar with the premises, having represented David and
Eizabeth Parrish when they purchased the premises from Alice W. Strickland
on September 4, 1956. The property had previously been owned for some years
by Mrs. Strickland and her husband Chester E. Strickland, who purchased it
from the Riverhead Savings Bank in 1941. The residence is a victorian type
residence which was constructed probably many years prior to the turn of the
century and the shed or barn is of the same age. The Parrish family has done
nothing to improve the residence or barn on the outside, both are in the same
condition that they were when the Parrishes bought it in 1956 and probably
had not changed much if any prior to that time.
Deponent makes this affidavit to the Town of Southold to issue a
Certificate of Pre -Existing Use for the residence and

APPLICATION FOR CERTIFICATE OF OCCUPANCY
Instructions
A. This application must be filled in typewriter OR ink, and submitted in duplicate to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal—(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non -conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of peoperty showing all property lines, streets, buildings and unusual natural or
topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.
C. Fees:
