APPLICATION FOR CERTIFICATE OF OCCUPANCY
Instructions
A. This application must be filled in typewriter OR ink, and submitted in triplicate to the Building
Inspector with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and
unusual natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal—(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and
installations, a certificate of Code compliance from the Architect or Engineer responsible for
the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non -conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of property showing all property lines, streets, buildings and unusual natural
or topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent in-
formation required to prepare a certificate.
C. Fees:
1. Certificate of occupancy $5.00
2. Certificate of occupancy on pre-existing dwelling or land use $5.00
3. Copy of certificate of occupancy $1.00

Upon request of the Southold Town Building Department,
I made inspection of this two story framed dwelling and
found the following violations of Local Law #1, Housing Code
Town of Southold. I picked up key from Dr. Zitek's office,
Main Road, Southold, and let myself into the west entrance
beginning inspection at approximately 1:30 ap.m
The building does not have a central heating system
and cannot be occupied from November 1st to May 1st -
Section 513a.
FIRST FLOOR
Half Bath: Northwest corner of building, no light
fixture in toilet room - Section 215a. Three broken
and missing window panes - Section 302b. Room not
furnished with moisture proof flooring - Section 303d.
and Section 214d.
Dining Room: East side, Bay section, excessive sag to
point where window sash does not set securely in frame -
Section 301 a, b. Three broken window panes - Section
302b.
Bedroom: Southeast, paint excessive peeling off walls
and ciling - Section 303c.
Dressing room: Broken window pane - Section 302b.
Kitchen: Gas range, no shutoff valve at unit - Section
508b. Broken pane of glass window over sink - Section
302b. north end of kitchen floor sagged and excessively
springy - Section 301 a and 303a.
SECOND FLOOR
Stairway: No light to allow for safe ascent or descent -
Section 215c.
Hallway walls: c Plaster loose and cracking - Section 303c.
Bedroom: Southeast corner, plaster, walls, ceilings
cracking - Section 303c.
Bedroom: Southwest - cracked ceilings - Section 303c.
Hallway: North end of building, ceiling and walls plaster
off exposing lathe - Section 303c.
Shed attached to north end of kitchen, open chimney flue
Section 303b. Floor, walls and ceiling deteriorating -
Section 301c, 302b, c, and 303a.
Pantry, west side of building, plaster loose and falling
off ceiling and walls - Section 303c.
Hallway between kitchen and pantry, floor rear entrance,
excessive spring and rotting - Section 301a.
Cellar: Partial, outside entry, doorway deteriorating -
Se tc ion 301a, c. Southeast corner of cellar room with
ceiling plaster loose and falling away from lathe -
Section 303c. Cellar window units, broken panes of
glass - Section 302b.
Exterior: Porch, near front entry, boards excessively
springy and deteriorating - Section 301a. Foundation
under kitchen and shed north end of building, crawl

Inspection completed at approximately 2:45 p.m. Returned
key to Dr. 2iteks office, left with receptionist.
