Upon request of the Southold Town Building
Department I made inspection of this two story,
two family masonary dwelling and found the
following violations of Local Law #1, Housing
Code of the Town of Southold. I began inspection
at approximately 10:05 a.m. and was admitted to
front entrance and -directed to first floor apart-
ment by Mrs. Auzins:
FIRST FLOOR APARTMENT
Entrance to hallway of apartment - no switch
to control light - Section 529b.
Kitchen - Rear, outside entrance, no switch
to control light in room on entry - Section
529b.
Living Room - Broken window pane - Section 302b.
SECOND FLOOR APARTMENT
Kitchen - Gas range, no shutoff at unit -
Sect o 508b.
Cellar - Outside access only, no light switch
to control light at entry - Section 529b.
Storage Shed (lean to)- north side of building -
open junction box - Section 528a.
Inspection completed at japproximately 10:45 a.m.

AFFIDAVIT
HAROLD LEVINE and FLORENCE LEVINE, his wife, being
,jointly and severally sworn, depose and say:
THAT they are the purchasers under a contract between
'Tetsuo Ochikubo and Jeanie Teruye Ochikubo, his wife, as sellers,
',and your Deponents, as purchasers, of certain property located on
'the north side of Middle Road, Mattituck, Town of Southold, Suffolk
II;e,County, New York; and
THAT your Deponents have been advised by Mr. Howard
Terry, Building and Zoning Inspector of the Town of Southold,
that the building on the subject premises is presently being
utilized as a two-family dwelling, in violation of the zoning
ordinance of the Town of Southold; and
THAT your Deponents covenant and agree that upon the
expiration of the lease on the subject premises, which lease ex-
pires March 1, 1972, to eliminate the two-family use; and
THAT your Deponents further covenant and agree that
they will not utilize the said premises for their own use so as
to occupy the house as a two-family dwelling, until the premises
have been reconverted into a one -family dwelling; and
THAT your Deponents make this Affidavit to induce the
;Building and Zoning Department of the Town of Southold to issue
a conditional Certificate of Occupancy for a single-family dwell-
ing, and to further issue a Certificate -of Compliance that the
'building conforms to the housing code of the Town of Southold.

Instructions
A. This application must be filled in typewriter OR ink, and submitted in triplicate to the Building
Inspector with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and
unusual natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage dispcsal—(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and
installations, a certificate of Code compliance from the Architect or Engineer responsible for
the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non -conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of property showing all property lines, streets, buildings and unusual natural
or topographic features.
2. Sworn statement of owner or previous owner as to use,. occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent in-
formation required to prepare a certificate.
C. Fees:
1. Certificate of occupancy $5.00
2. Certificate of occupancy on pre-existing dwelling or land use $5.00
3. Copy of certificate of occupancy $1.00
