Instructions
This application must be filled in typewriter OR ink, and submitted m ~ to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal--(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
For exJsting buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of pz~perty showing all property lines, streets, buildings and unusual natural or
topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.

WILLIAM J. MILLS III, being duly sworn, deposes and says:
I reside at 316 Fourth Street, Greenport, New York, and
am the present owner of premises located at Corwin and Ninth Streets,
Greenport, New York, designated on the Suffolk County Tax Map as
District 1000, Section 048, Block 02, Lot 001, and am familiar with
the condition of same;
That the structures on the premises have been designed,
arranged and have been physically used as follows: As a warehouse with
detached garage and fences for the manufacturing, assembling, handling
or storage of products or materials, truck terminal, parking facilities
loading docks, boat servicing, and boat storage;
That this affidavit is made for the portion of the
premises constructed prior to April 23, 1957; and that the condition
of same is good;
That I make this affidavit in the knowledge that the
Building Department of the Town of Southold will rely upon the accuracy
hereof in issuing a certificate of occupancy for pre-existing use.
