New York 11743 Re: 'Application No. 597 - 1000-035-04-19 Dear Mr. Raymes: The following action was taken by the Board of Town Trustees during their regular meeting held on January 21, I988 regarding the above matter. Moved by Trustee Bredemeyer seconded by Trustee Bednoski WHEREAS, Frederick Raymes applied to the Southold Town Trustees for a permit under the provisions of the Wetland Ordinance of the Town of Southold, application dated October 15, 1987, and WHEREAS said application was referred to the Southold Town Conservation Advisory Council for their findings and recommendations, and WHEREAS, a public hearing was held by the Town Trustees with respect to said application on January 21, 1988 at which lime ail interested persons were given an opportunity to be heard, and WHEREAS, the Board members have personally wewed and are familiar with the premises in question and the surrounding area, and WHEREAS, the Board has considered all the testimony and documentation submitted .concerning this application, and WHEREAS, the Board has determined that the project as proposed will not affect the health, safety and general welfare of the people of the town, NOW, THEREFORE, BE IT RESOLVED that Frederick Raymes BE AND HEREBY IS GRANTED PERMISSION UNDER .THE PROVISIONS OF THE WETLAND ORDINANCE OF THE TOWN OF SOUTHOLD TO: Construct a one family dwelling with a deck and tool storage building. Property is located at 704 Wiggins Lane, Greenport. This permit is approved subjec~ to the prevention of any building debr is and/or run off during the construction phase and the installation of dry wells for th~ finished house. There are two inspections required and the Trustees are to be notified upon the completion of the work. Prior to the issuance of said permit by the Clerk, the applicant shall file with the Clerk a certificate that he has public liability insurance policies insuring against any liability which may arise in the performance of the operations pursuant to such permit ~n such amount as shall be fixed by the Trustees, which said policies shall name the Town as a name insured. Please take further notice that this declaration should not be considered a determination made for any other department or agency which may also have an application pending for the same or similar project. This application is to be completely filled in by typewriter or ink and submitted to the Town Clerk in quadruplicate, accomoahied by written permission from the owner of the property if not the same as the applicant. B. Pilot Plan must be drown to scale on the reverse side of this application showing the |acotion of property, the names and ]ooat~ons of owners of adjoining properties, and o detailed description of proposed ~roject. (Note: Copies of the Pilot Plan may be made by a copy machine and attached to the Permit Application.) C. No operation shall be initiated by the applicant until all permits that are required are issued. D. A detailed statement of the rehabilitation and proposed condition of the premises after the work is completed must be given, including a survey if required, E. If permit is being sought for a more sizeable excavation than the construction of a private single dock or jetty, then this application must be accompanied by a survey end topographical map certified by a registered land survey or Professional Engineer. The horizontal control of survey shall be based upon an approval local coordinate system. The vertical control for elevation and sound- ings shall be based upon United States Coast and Geodetic Survey and/or United States Geological Survey datum, F. A statement must accompany the application describing any known prior operations conducted on the premises in question and whether any prior licenses or permits have been issued to erect structures or to dredge or deposit fill on said premises and whether any such permits or licenses were ever revoked or suspended by a governmental agency. G. A filing fee of $25.00 shall accompany the application. This filing fee inclucl~s one inspection, there is o $5.00 fee for each additional inspection. H.
