has been conducted. This Certificate, therefore, does not, and is not intended

JONATHAN D. BROWN, being duly sworn, deposes and says:
(1) That I am an associate in the law offices of TOOKER AND
SMITH, ESQS., attorneys for NORTHVILLE INDUSTRIES CORP., record owner
of the premises located at Westphalia Avenue, Mattituck, New York. Tax
Map No.: 1000-141.00-03.00-035.000, and as such, from a review of the
file and from many visits to the premises mentioned, I am fully
familiar with all the facts and circumstances heretofore had herein.
(2) That I do make this Affidavit at the request of the
Building Department of the Town of Southold in support of our applica-
tion for a copy of the Certificate of Occupancy and/or Certificate of
Pre-Existing Use covering the buildings and premises.
(3) I refer to the survey annexed hereto as "Exhibit A" and
make the following comments with regard to the structures located on
the premises, and numbered for convenience of reference, Structures 1
through 4:
(a) Concrete Block Building No. 1 was used formerly as
an office for North Fork Oil Heat, Inc. with attached garage. The con-
