INSTRUCTIONS: Fill out form below and return to above address. You
should have this application approved and returned to you within a week°
3; The purpose of this application: Redriving or replacinq openwork
dock or openwork pier piling, and mooring piles, removal of damaged
piling~ repair, resetting or replacement of floating docks, ramps,
or decking, all necessitated by ice and storm damage resulting during
1987-1988 winter.
SPECIAL CONDITIONS
1. No dredging or filling is permitted.
2. Repairs to existing structures shall be limited to replacement of
timbers without change in design or dimension.
3. No repair or replacement of bulkheads is permitted under this permit.
4~ No freshwater or tidal wetlands are to be disturbed under any
circumstances. ~-~----~
..5. A yellow permit siTn provided by this office shall be displayed on
the project at all times while under replacement.
6. Original permit numbers (3 '~ high)::must be permantty displayed on
project.

estimated time required to complete the work involved, but should circumstanc~ warrant~ request
for an ex-tension may be made to the Board at a later date.
3. That this Permit should be retained indefinitely, or as long as the said Permittee wishes
-' to maintain the structure or project involved, to provide evidence to anyone concerned that anth-
orization was originally obtained.
4. That .the work involved will be subject to the inspection and approval of the Board or
--irs agents, and non-compliance with the provisions of the originating appllcation~ may be cause for
revocation of this Permit by resolution of the said Board.
5. That there will be no unreasonable interference with navigation as a result of the work
herein authorized.
6. That there shall be no interference with the right of the public to pass and repass along
the beach between high and Iow water murks.
-- 7. That if furore oberatioos of the Town of Southold require the removal and/or alterations
in the location of the work herein authorized, or if, in the oFiulon of the Board of Trusre~, the
work shall came u~reasonable obstrumoo to free navigation, the said Permittee will be required,
upon due notice, to remove or alter this work or project herein stated without expenses to the Town
of Southold.
8. That the said Board will be notified by the Permlttee o~ the completion of the work a~th-
orized.
9. That the Permktee will obtain all other permits and. comenrs that may be required
plemental to this permit which may be subject ro revoke upon failure to obtain same.

Dear Mr. Olmsted:
The following action was taken by the Board of Trustees
at their regular meeting held on April 24, 1986 regarding your
application.
Moved by Trustee Krupski seconded by Trustee Goubeaud it was
RESOLVED to APPROVE the application submitted by Sidny Olmsted
for the placement of two spiles in Mattituck Creek, Mattituck
to be used to hold off boats from the existing dock. This permit
is approved with the provision that if there is a problem with
navigation, determined by the Bay Constable, that one spile will
be removed.
The following fee is now due and payable. Upon remittance
your permit will be forwarded by mail.

RESOLVED that the Town Trustees APPROVE/D~NY the application
submitted by Sidny Olmsted, residing at 1300 Westphalia Road,
Mattituck to drive 2 spiles in Mattituck Creek. These spiles
will be used to hold off boats moored to his dock.
Please note that the Bay Constable, Don Dzenkowski has advised
that there is no problem with the spiles, but when he wants to
tie up 50' clam boats, that there will be a problem with
moving the boat in and out.
