2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement fi'om plmnber certifying that the solder used in system contains less than 2/t0 of 1% lead.
5. Commercial building, industrial building, .multiple residences and sinfilar buildings and installations, a certifica!
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.

Kay Mason, being duly swom, depose and say that:
1. I presently reside at 345 Holden Avenue, Cutchogue, NY 11935.
2. I am very familiar with the premises known as 645 holden Avenue, Cutchogue
NY 11935, having lived in the vicinity since the early 1950s.
3. I have personal knowledge of the existence of a residential home as depicted on
a survey prepared by John C. Ehlers Land Surveyor, dated 6/20/02.
4. Said house existed on the property prior to April 9, 1957 in substantially the
same form as depicted in the attached survey, and I know this to my own knowledge.
5. The house has been used continuously for residential purposes as a one family
dwelling since that time up to the present.
6. I make this affidavit knowing fully that the Town of Southold Building
Department will rely upon the facts as stated herein to issue a pre-existing Certificate of
Occupancy for said structure(s) to Salvatore M. Catapano, Jr. as a one family dwelling.
