This application must be filled in typewriter OR ink, and submitted in duplicate to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approv,a_i of completed site plan requirements where applicable.

JOSEPH E. NOLAN, being duly sworn, deposes and says:
1. That he is one of the owners of premises situate at
Main Road and New Suffolk Lane, Cutchogue, NY, and as more particu-
larly described in the annexed photocopy of survey of premises
dated March 30, 1979.
2. That I make this affidavit in an effort to secure a
Certificate of Occupancy on a pre-existing commercial building,
containing four (4) street level stores and two (2) residential
rental apartments on the second floor, for the premises above
described in paragraph "1".
3. The aforesaid premises and improvements thereon were
purchased on the 16th day of April, 1979.
4. That upon information and belief, the premises have
been used without change and for no other purpose than as a com-
mercial building containing four (4) street level stores and two (2)
residential rental apartments on the second floor, since the date
of their purchase.
5. That upon information and belief, the premises have
not been used for any other purpose than permitted by the Zoning
Ordinance of the Town of Southold.
6. That said pre-existing Certificate of Occupancy
should be issued in the names of Joseph E. Nolan and Anne T. Nolan.
